What the data is — and isn't

All information on this Site is derived from public sources, including: NPPES NPI Registry, CMS Medicare Utilization & MIPS data, CMS Hospital Compare, OIG List of Excluded Individuals/Entities (LEIE), NY State Medical Board, NY OMIG, DEA public records, PubMed, NIH Reporter, ClinicalTrials.gov, Semantic Scholar, and Google Reviews. No proprietary or non-public data is used.

Public data has real limitations: it may be incomplete, outdated, or reflect only a subset of a provider's practice (for example, CMS data captures Medicare patients only). Scores and tier labels are algorithmic outputs based on disclosed methodology — they are opinions, not factual determinations of quality.

A listing on this Site is not an endorsement. The absence of a listing does not imply a negative finding. A high score does not guarantee a good outcome. A low score does not mean a provider is unsafe.

Disciplinary and exclusion records

When this Site displays a disciplinary flag or federal exclusion, it is reproducing a public government record. We do not independently investigate or adjudicate disciplinary matters. Federal OIG exclusion data is sourced directly from the OIG LEIE and can be independently verified at oig.hhs.gov.

Exclusion status changes over time. A provider may have been reinstated since a record was flagged. Always verify current status directly with the OIG and the NY State Medical Board before making any scheduling or contracting decisions based on this information.

Data accuracy and corrections

We make reasonable efforts to match records accurately using government-issued NPI numbers and other identifiers, but errors can occur — including incorrect matches, stale records, and data inherited from source databases.

If you are a provider and believe your profile contains an error, or if you are a user who believes a profile is incorrectly attributed, contact us at data@provdr.co with the provider NPI, a description of the error, and (if available) documentation. We will investigate and correct confirmed errors promptly. Sanchin Advisory LLC is not liable for damages arising from data errors that were not previously reported to us.

No warranty; limitation of liability

Because this Site is provided free of charge, our total liability to you for any claim shall not exceed zero dollars ($0).

Intellectual property

The scoring methodology, software, design, and original written content on this Site are owned by Sanchin Advisory LLC. Underlying government data remains in the public domain. You may not scrape, reproduce, or redistribute our scores, tier labels, or compiled provider profiles for commercial purposes without written permission.

Governing law and disputes

These terms are governed by the laws of the State of New York, without regard to conflict of law principles. Any dispute arising from your use of this Site shall be resolved exclusively in the state or federal courts located in New York County, New York, and you consent to personal jurisdiction there.

Changes to these terms

We may update these terms from time to time. The "last updated" date at the top of this page will reflect any changes. Continued use of the Site after an update constitutes acceptance of the revised terms.
